Case Study: PCOS, Insulin Resistance, and Muscle Growth
Patient Profile:
Sarah, a 29-year-old woman that has been diagnosed with PCOS (Polycystic Ovary Syndrome). She came to me with several health concerns related to her condition, including insulin-resistant PCOS, weight gain, fatigue, and difficulty building muscle. Also, her increased weight gain led her to joint pain and knee instability.
Objective Assessment:
During the initial assessment, I noted the following:
- Decreased of the muscle mass, especially in her lower limbs.
- Poor ankle stability and a knee valgus when performing squats.
- Elevated insulin levels, she was borderline prediabetes, and had low vitamin D levels.
Intervention Plan:
Weeks 1-6 – Foundational Strength & Metabolic Conditioning
The first phase of my program focused on establishing consistency, which is often challenging for women with PCOS. The primary goal was to enhance her insulin sensitivity and build her strength.
- Strength training: 3 times per week, using compound movements like squats, deadlifts, push-ups, and bent-over rows. I placed a focus strengthening her posterior chain to help with Sarah’s knee instability.
- Cardiovascular training: Low-impact steady-state light cardio 3 times a week, with additional HIIT sessions (30 seconds of work, 30 seconds rest) in order to improve her insulin sensitivity.
- Nutrition: A whole-food, balanced macro approach was implemented with the help of Francine, my favorite PCOS dietitian specialist, in order to priorities insulin-friendly foods, such as lean proteins, fibre-rich vegetables, and complex carbs.
- Intermittent fasting /Time-restricted eating : Intermittent fasting allows to not trigger insulin peacks during the day, eating only during a pre-determined window of time during the day helps to achieve this goal and improve insulin management without privation. Exp; you can eat only during a 8h window that suit your personal schedule. Exp: you can eat from 1pm to 9pm and the rest of the day you don’t.
- Physiotherapy: I used soft tissue release for her tight hip flexors and her lower back, along with mobility drills for her knee and ankle stability. Education on sleep, stress management, and hydration was also provided.
Results after Weeks 1-6:
- Weight loss: 4kg.
- Improved knee stability and better squat mechanics.
- Increased energy levels and no longer experiencing fatigue post-workout.
- Reduced sugar cravings and improved blood sugar control.
Weeks 7-12 – Strength and Muscle Growth
The focus during this phase shifted to a progressive overload, aiming to build her muscle mass and enhance her metabolic efficiency.
- Strength training: Increased to 4 times per week, with heavier weights (about 70% of 1RM) and higher volume (3-4 sets per exercise).
- Plyometrics and kettlebell circuits were introduced twice a week to increase intensity.
- Nutrition: Protein intake was increased to support muscle growth, and carb cycling was incorporated to optimize insulin management.
- Physiotherapy: Breathwork strategies for cortisol control and sleep hygiene practices were introduced to support recovery.
Results after Weeks 7-12:
- Total weight loss: 9kg.
- Increase of her muscle definition and significant strength gains.
- Improvement of her insulin sensitivity and reduced carb cravings.
- Enhancement of her mood and she gained a more stable energy levels.
Weeks 13+ – Long-Term Sustainability and Refinement
At this stage, Sarah had made great progress, and my focus was on sustainability. We aimed to refine her movement efficiency and helped her maintain long-term lifestyle changes.
- Strength training continued 4 times per week, with an emphasis on athletic-style training.
- A Habit tracking and a mindful eating approach were incorporated to improve her sleep quality and stress management.
