What are the causes of Elbow Pain?

Elbow pain can come from a vast list of causes, some of the most common causes of elbow pain are: Repetitive motion of the arm, overuse of the elbow joint, poor biomechanics, improper technique during sports, trauma or accidents, direct impact on the elbow or falls on the elbow, heavy lifting, twisting motions, excessive stretching or tension badly done, degenerative wear and tear, sports involving repetitive throwing (e.g., golf, tennis), joint instability, hyper laxity of the elbow joint, aging, inflammatory conditions (e.g., rheumatoid arthritis), previous elbow injury, or poor posture.

The Elbow: A complex anatomy, yet efficient

The elbow joint is a unique and highly functional structure where the humerus (upper arm bone) connects with the radius and ulna (forearm bones). The elbow works as both a hinge joint, allowing the flexion and extension of the arm, and as a pivot joint, which facilitate the pronation and supination (rotation of the forearm). This combination of motions makes the elbow both flexible and stable. Due to its intricate anatomy, the elbow is often more prone to stiffness  following an injury.

Range of Movements at the Elbow

Flexion and Extension:
Flexion and extension are taking place at the ulnohumeral joint. Typical the range of motion (ROM) is between 0° to 140°, though most daily activities require only 30° to 130° of movement.

Pronation and Supination
Pronation and supination are controlled by the radiocapitellar joint and proximal radioulnar joint. The normal range of motion for these movements is approximately 180°, with 80°-90° for pronation and 90° for supination. A 50° range of pronation and supination is generally sufficient for most daily tasks. Loss of pronation can be compensated by shoulder abduction, but the inability to supinate effectively has no clear compensation, making it a loss of supination ROM potentially more disabling than a loss of pronation.

Elbow pain is rarely isolated. Multiple structures can refer pain to the elbow or contribute to its development. Research has shown that dysfunctions in areas which are distant from the elbow plays a significant role in elbow pains and dysfunctions. These contributing factors can be neural, myofascial, joint-related, or even centrally mediated.

Shoulder, Cervical Spine, and Thoracic Spine can trigger Elbow pain

Both the cervical and thoracic spine have been identified as influencing factors in elbow pain. Studies have demonstrated that individuals with lateral elbow pain often experienced pain in these areas. For example, “Berglund et al”. found that 70% of  the patients with a lateral elbow pain also reported a pain in their cervical or thoracic spine, compared to only 16% in the asymptomatic group. In addition, “Zunke et al”. showed that thoracic spine mobilisation improves pain free grip strength for patient with a lateral epicondylalgia.

Now a weakness in the scapular muscles, mainly in the serratus anterior and lower/middle trapezius, is another significant risk factor for developing elbow pain. Fatigue or weakness in some of all of these muscles can alter upper limb biomechanics, leading to dysfunction at the elbow. “Lucado et al. (2012)” found out that female tennis players with lateral epicondylalgia had weaker wrist extensors and lower trapezius muscles compared to some asymptomatic players.

Compensatory movements of the elbow occurs as a result of the dysfunction in other joint complexes. For exemple, a loss of the glenohumeral lateral rotation can lead to increased supination of the forearm or a valgus as compensation. in the mean time a loss of glenohumeral internal rotation may increase forearm pronation. These compensatory movements increase the stress on the elbow structures and also contribute to pain and dysfunction.

The underlying issue of other joints reduced mobility will also need to be treated to alleviate stress on the elbow. Local Examination of the Elbow:

  • Active and Passive Movements – the physio will assess the range of motion (ROM) both actively and passively to evaluate flexibility and joint function.
  • Muscle Strength Testing – the physio will be testing the strength of the muscles around the elbow joint in order to detect some weaknesses or imbalances that could contribute to an elbow dysfunction.

  • Grip Strength Measurement – will check the strength of your grip to assess hand and forearm strength.
  • Palpation and Manual Examination of the elbow– the physio will press on the elbow and elbow mucles to identify tenderness, swelling, or abnormalities
  • Thermal Hyperalgesia – The physio will be evaluating changes in the temperature  and the sensitivity of the elbow area to check for any inflammation or nerve irritation, which can contribute to your elbow pain.

  • Cervical Spine – the physio will assess your neck to determine if there is any referred pain from the cervical spine that could affect the elbow.
  • Thoracic Spine – the physio will be checking for any dysfunction in the upper back that may contribute to some shoulder or elbow conditions.
  • Shoulder – The physio will evaluat your shoulder mobility and strength. It is not rare that a shoulder dysfunction often causes compensatory stress on your elbow.
